Material of Armored Tracheostomy Tube

Metal tubes are constructed of silver or stainless steel. They are not used commonly due to expense, rigid construction, and lack of a cuff and a 15-mm connector to attach a ventilator. A smooth rounded-tip obturator passed through the lumen of the armored tracheostomy tube facilitates insertion of the tube by displacing rather than coring tissues; it is removed once the tube is in place. Metal is durable, inhibits bacteria growth, does not react with tissues, resists biofilm formation, is easy to clean, and can be sterilized with heat or steam.

The most commonly used armored tracheostomy tubes are made from polyvinyl chloride (PVC), silicone, or polyurethane. PVC softens at body temperature, conforming to patient anatomy and centering the tube in the trachea. Silicone is naturally soft and unaffected by temperature, resists colonization and biofilm buildup, and can be sterilized. Some armored tracheostomy tubes are packaged with a tracheal wedge, which facilitates removal of the ventilator circuit while minimizing the risk of dislodgement of the armored tracheostomy tube. Like the metal tube, an obturator facilitates insertion of the tube.

Outer cannula: Main body of the tube that stays in the trachea until the tube is changed.

Neck flange: The flat part that rests against the neck, securing the tube. Contains manufacturer’s name, tube size, and presence of a cuff and/or fenestration.

Eyelets: Holes on the sides of the neck flange where ties are inserted to secure the tube.

Cuff: An inflatable balloon at the end of the outer cannula used to seal the space between the tube and wall of the trachea.

Fenestration: Opening(s) or hole(s) in a cannula.

Pilot balloon: Indicates cuff inflation and serves as a port for inflating/deflating the cuff. When the cuff is inflated, the pilot balloon is also inflated.

Inner cannula: A removable tube inside the outer cannula for cleaning.
