Benefits of Guedel Airway
01.

Simplicity and ease of use:

These are simple to use even in emergencies, and a minimum amount of training is required for their effective deployment.

02.

Cost-Effectiveness

Guedel airways are relatively inexpensive compared to other airway devices, making them a cost-effective solution for airway management.

03.

Versatility

It is suitable for a wide range of patients, from neonates to adults, and it can be used in several settings, such as pre-hospital, emergency departments, operating rooms, and intensive care units.

04.

Facilitates Effective Ventilation

They help keep the airway patent, which in turn allows for more effective ventilation either manually or with the use of a ventilator.

Main Components of the Guedel Airway

The Flange
The flange on the Guedel airway is wide and rigid. It prevents the device from being inserted too far into the oropharynx. Meanwhile, it offers a secure surface for clinicians to grip. The flange might have indicators for size. It helps distinguish adult from pediatric variations. Most designs have a smooth rim to decrease tissue trauma. Some flanges are color-coded to help categorize length at a glance. It’s handy during rapid airway management since the wrong size may worsen obstruction rather than improve it.

The Airway Channel
The Guedel airway’s central channel is the airflow conduit. It extends from the flange end to the distal tip. The channel is elliptical for lower internal resistance. It permits the passage of air, oxygen, or anesthetic gases.

The Biting Block
The biting block is between the flange and the channel. It withstands occlusive force from a patient’s jaws. The component protects the teeth and the device itself.

The Distal Tip
The distal tip of the Guedel airway sits in the oropharynx behind the base of the tongue. It is angled or curved to lift the tongue away from the posterior pharyngeal wall. The shape depends on the design.
A well-curved tip can decrease contact with the epiglottis and uvula. It also lowers trauma risk if the device is advanced. The distal tip must be smooth to circumvent mucosal injury. Some Guedel airway models have a reinforced tip to help guide suction catheters. Others have extra channels on the tip to adjust airflow around the tongue.

Usage Method

The tube is sized by placing it vertically, with the flange in the middle of the lips and the tip emerging at the angle of the mandible.

In adults it is inserted into the mouth upside-down, and rotated into the normal position once it is inserted about half way.

Lubricant may be useful if the mouth is excessively dry.

FAQ

Q: When may you need to insert an oropharyngeal airway?

A: Use an oropharyngeal airway only if the patient is unconscious or minimally responsive because it may stimulate gagging, which poses a risk of aspiration. Nasopharyngeal airways are preferred for obtunded patients with intact gag reflexes.

Q: Why is Guedel inserted upside down?

A: In adults, the Guedel airway is initially inserted upside down, with the curvature facing caudad. Once partially inserted, it is then rotated through 180° and advanced until the bite block rests between the incisors. This method prevents the tongue being pushed back into the pharynx, causing further obstruction.

Q: What is a Guedel airway used for?

A: An oropharyngeal airway (also known as an oral airway, OPA or Guedel pattern airway) is a medical device called an airway adjunct used in airway management to maintain or open a patient's airway. It does this by preventing the tongue from covering the epiglottis, which could prevent the person from breathing.

Q: How do I choose a Guedel airway?

A: Determine the appropriate size of the oropharyngeal airway. Hold the airway beside the patient's cheek with the flange at the corner of the mouth. The tip of an appropriately sized airway should just reach the angle of the mandibular ramus.
